Case Management

Helping people with disabilities and
their families get the services they need.

ARC stands for Advocacy, Resources and Commitment. Our Case Management Program is for adults (age 18 and older) with developmental and related disabilities. This program’s goal is to help individuals and, when appropriate, their families access and coordinate a full spectrum of services that includes medical, social, educational, vocational and more.

You can rely on ARC Case Management to:

  • Complete a comprehensive assessment
  • Develop an Individual Care Plan
  • Provide ongoing monitoring and support
  • Coordinate services

Two service options.

  1. Targeted Case Management, which is billable to Medicaid.
  2. Fee–for–service Case Management.

These two types of services provide very similar kinds of assistance to the client; however, there is one key difference. Targeted Case Management is only available to those who have Title–19 (Medicaid) and have been referred to ARC’s Program by the Milwaukee County Disability Services Division.

For those individuals who do not qualify for government assistance, ARC offers fee–for–service programming. We carefully assess the needs of each individual to determine which services are needed and which are not. This tailored and considered approach insures that individuals only access and pay for services that are needed. It’s a thoughtful approach that makes wise use of funds available from the individual, their family members or a trust.

  • Act on their own behalf as much as possible.
  • Participate to the fullest extent in:
    • Community activities they choose.
    • Establishing community supports.
    • Creating their Individual Care Plans.
  • Develop or regain their capacity to meet the essential requirements for physical health and safety.
  • Develop their informal support network.
